Specific Internet Project Permission Slip

Dear Parent,

You have probably heard about the History Fair Project going on now in the fifth grade. Asa Adams Library has quite a good collection of materials but IÍm aware that with 92 (!) fifth grade students, that our print material won't stretch endlessly. Also I found, during last yearÍs project, that students are developing excellent research questions which are not easily answered! Therefore we are asking permission for your child to use the Internet (World Wide Web) under supervision*.

Because Internet research is an area of intense interest to teachers and librarians around our state, I would also like permission to videotape or audiotape your child for purposes of showing other educators about this educational activity. To protect your family's privacy your child will not be named. If you chose not to allow your child to be taped, he or she will still be able to perform ALL the homework and research needed to complete the History Fair Project.

* Supervised means that the project will be overseen by adults; not that someone will be constantly watching over a student's shoulder all the time.
